Drug Dose Adjustment in Chronic Renal Diseases
نویسنده
چکیده
The development of numerous drugs over the last two decades to a variety of diseases with significant benefits has paralleled increasing awareness of the adverse effects of these drugs. Majority of the drugs are eliminated by the kidneys. Therefore it is mandatory to make appropriate dosage modification when renal function deteriorates to prevent drug toxicity. Dosage modification tables of large number of drugs that are affected by chronic kidney disease (CKD) are available in medical literature and they are also provided by the pharmaceutical industry. The action of a drug is related to its free form concentration in the tissues. The bio-availability of the drug depends on the route of administration, absorption factors in the gut and bio-transformation in the liver.

The Rate of Antibiotic Dosage Adjustment in Renal Dysfunction
The purpose of this study was to determine the number of prescribed antibiotics being appropriately adjusted and to assess antibiotics with the highest incorrect dosing based on the patient’s renal function according to distinguished guidelines. The study was conducted at a 446-bed university hospital. One hundred and fifty patients admitted through different wards of the hospital were included...

متن کاملDrug therapy in patients with chronic renal failure.
BACKGROUND Roughly 20% of patients in hospital have impaired kidney function. This is frequently overlooked because of the creatinine-blind range in which early stages of renal failure are often hidden. Chronic kidney disease is divided into 5 stages (CKD 1 to 5). METHODS Selective literature search. RESULTS Methotrexate, enoxaparin and metformin are examples of drugs that should no longer ...
متن کاملKidney function and clinical recommendations of drug dose adjustment in geriatric patients
BACKGROUND In elderly patients chronic kidney disease often limits drug prescription. As several equations for quick assessment of kidney function by estimating glomerular filtration rate (eGFR) and several different clinical recommendations for drug dose adjustment in renal failure are published, choosing the correct approach for drug dosage is difficult for the practitioner. The aims of our s...
متن کاملImpact of a renal drug dosing service on dose adjustment in hospitalized patients with chronic kidney disease.
BACKGROUND Appropriate drug selection and dosing for patients with chronic kidney disease (CKD) is important to avoid unwanted drug effects and ensure optimal patient outcomes. OBJECTIVE To assess the rate of inappropriate dosing in patients with CKD in a nephrology unit and to evaluate the impact on dose adjustment, adverse drug events (ADEs), and drug cost of having a pharmacist accompany a...
متن کاملDiltiazem Co-Treatment in Renal Transplant Patients Receiving Cyclosporine with Respect to Concentration at Two Hours (C2)
Cyclosporine (CsA) is a drug that has been used for prevention of kidney transplant rejection for many years. Diltiazem with the inhibition of cyclosporine metabolism and clearance will increase CsA concentration and CsA dose can be decreased. The aim of this study was the evaluation of diltiazem effect on CsA dose adjustment with respect to C2. Forty stable renal transplant p...
